COVID-19 Pre-screening for DBC offices

 

This pre-screening form must be completed and submitted before arriving at Destination BC’s (DBC) offices. This form must be completed on the day of your visit as close to the time of your visit as possible. A confirmation email will be sent to you and DBC’s People and Development Department (Human Resources) after you submit the pre-screening form.

NOTE: Please also check your spam folder for confirmation email.

As per DBC’s Proof of Vaccination Policy, effective December 13, 2021 all parties entering DBC’s offices must provide proof of full vaccination for COVID-19 before entering. Visitors will be required to provide proof of full vaccination to the DBC meeting host or other applicable designate in order to enter the offices. Any person who is unable to provide proof of full vaccination or with symptoms, or suspected COVID-19 symptoms, is not permitted to enter DBC’s offices.

Masks are a personal choice. Hand sanitizer stations and masks are available at our front doors and throughout the office.

**If you have answered NO to question 1, you will not be permitted access to DBC’s Offices. If you have answered YES to questions 2-3, we ask that you reschedule your visit until you are better. Thank you**
